چکیده

Abstract Background Fulminant Myocarditis (FM) leads to cardiogenic shock with multi-organic dysfunction, being peripheral venoarterial extracorporeal membranous oxygenation (VA-ECMO) a rescue technique. Despite growing experience, it is still related several complications. We aim appraise the management of patients FM and adverse events associated VA-ECMO. Methods conducted retrospective study in ECMO centre Portugal, which included 15 diagnosed managed on VA-ECMO from 2008 2018. Hemorrhage or infection cardiac device site, tamponade, limb ischemia, ischemic stroke, cerebral hemorrhage, severe hemolysis accidental decannulation were considered major Results For sample characteristics resumed complications, see Table. Inotropic mechanical ventilation support transversal all, only one patient extubated before decannulation. Six received renal replacement therapy, whom half recovered function remaining died. operated average for 8,5 days (range, 1 20 days). 3 had concomitant Impella heart pump them third assistance Intra-Aortic Ballon pump. Major complications observed 9 (60%), stroke most common. Limb 5 patients, longer duration (limb ischemia vs non-limb ischemia; 12,8±4,9 vs. 6,8±6,5 Two presented hemorrhage all fatal outcome. Infection not observed. Conclusion The common both apparently mortality. Differently, site tamponade less frequent but ominous Overall, can be serious hinder recovery discharge. Prognosis of patients with fulminant myocarditis managed by peripheral venoarterial extracorporeal membranous oxygenation support: a retrospective single-center study

BACKGROUND Peripheral venoarterial extracorporeal membranous oxygenation (ECMO) support is effective in patients with cardiogenic shock or fatal arrhythmia due to fulminant myocarditis. The clinical courses of fulminant myocarditis are still uncertain; therefore, it is difficult to determine the appropriate time for discontinuing ECMO or converting to a ventricular assist device. The purpose of...

Respiratory Syncytial Virus Associated Myocarditis Requiring Venoarterial Extracorporeal Membrane Oxygenation

Severe fulminant myocarditis causing cardiogenic shock can be a rapidly progressing, life threatening condition. Respiratory syncytial virus (RSV) is a very rare infectious culprit infrequently described in medical literature as a cause of myocarditis, particularly in adults. We present a case of acute fulminant myocarditis in a patient with PCR positive RSV infection requiring venoarterial ext...

Use of venoarterial extracorporeal membrane oxygenation in fulminant chagasic myocarditis as a bridge to heart transplant

A 17-year-old Brazilian male presented with progressive dyspnea for 15 days, worsening in the last 24 hours, and was admitted in respiratory failure and cardiogenic shock, with multiple organ dysfunctions. Echocardiography showed a left ventricle ejection fraction of 11%, severe diffuse hypokinesia, and a systolic pulmonary artery pressure of 50mmHg, resulting in the need for hemodynamic suppor...

Extracorporeal membranous oxygenation support for acute fulminant myocarditis: analysis of a single center's experience.

OBJECTIVES Acute fulminant myocarditis (AFM) is a disease category that is easily neglected. Circulatory mechanical support is sometimes required for this devastating condition. We analyzed our experience in managing AFM with mechanical circulatory support. METHODS We applied extracorporeal membrane oxygenation (ECMO) as a first-line rescue for AFM. The diagnosis was mainly derived from clini...

The first succesful extracorporeal membranous oxygenation treatment in a child with refractory fulminant myocarditis in Turkey

In acute fulminant myocarditis (AFM) with refractory medical treatment, the ventricular function returned to normal if venous–arterial ECMO (VA-ECMO) is applied early (1, 2). A previously healthy 26-month-old girl was brought to the hospital with a 3-day history of vomiting, fever, and tachypnea. Her Glasgow coma scale was 11, respiratory rate was 60/min, heart rate was 152/ min, blood pressure...
